Participant Donor Forms
You submit offline donations with a form specific to your training season. You got one of these forms in your Kickoff packet, and you can download extras below.
To distribute donations among members of a fundraising team, use a Team Donation Submission Form. Paycor does not keep track of team names, so you need to use the form to list the individuals on your team.
Forms for submitting offline donations:
What is Paycor?
The Leukemia & Lymphoma Society utilizes Paycor, an accounting center based in Ohio, to process all Team In Training contributions. Online donations are processed automatically; offline donations (checks, money orders, etc.) must be submitted to Paycor via mail with a Participant Donor Form.
Handling contributions
Instruct your contributors to send all offline donation (checks, money orders, etc.) directly to you.
Mail matching gift forms to your chapter office in San Francisco or Santa Rosa.
Mail contributions to Paycor early and consistently—don't wait! Credit cards and checks expire, so you want to send them to Paycor quickly. Sort your contributions by type and submit them as detailed here:
Checks
Checks should be made payable to "LLS". Write your name, team, sport and "Greater SF Chapter" (for SF, Marin and East Bay teams) or "RWC Branch" (for Redwood/Wine Country and Solano teams) in the memo section of each check, then simply batch and mail to Paycor with your Participant Donor Form.
- Do not staple or tape checks!
- Don't send post-dated checks. Hold these in your possession until the check date is valid.
- Don't send checks dated over three months old. These checks will not be accepted and will be returned.
- Please review checks to ensure they are signed and dated.
Credit Card Contributions
You should accept credit card donations through your eFundraising website if possible. Otherwise, complete the Credit Card Donation section of the Participant Donor Form for all credit card contributions. Write carefully and clearly, because errors will result in denied credit card approval. (Paycor can only process Visa, MasterCard, and American Express).
Cash: DO NOT SEND CASH.
Convert cash contributions and any foreign currency into a check or money order payable to LLS. To ensure your cash contributors get a tax receipt, complete the Cash Donation section of the Participant Donor Form. The Paycor Accounting Center will not be liable for any lost or stolen cash donations.
Foreign Currency
Your fundraising webpage is the best place to accept foreign donations. If that's not a possibility, you must convert any foreign currency or checks you receive into U.S. dollars before you submit them. Deposit the donations into your personal account and write a check for the amount in U.S. currency, then mail the check to Paycor. To ensure your foreign contributors get a tax receipt, complete the Cash Donation section of the Participant Donor Form.
Matching Gift Contributions
See Matching Gifts for more information. Make a copy of the corresponding donation and attach it to the matching gift form, and make sure your name, team, event and "Greater SF Chapter" (for SF, Marin and East Bay teams) or "RWC Branch" (for Redwood/Wine Country and Solano teams) are written on the form. Send the matching gift form and the copy of the donation to your chapter office. Mail the actual donation to Paycor.
Remember:
- Mail contributions to Paycor in Ohio.
- Mail matching gift forms and a copy the donation to your chapter in San Francisco or Santa Rosa.
Where do I mail my contributions?
First off, make sure you've completed these steps:
- Gathered your contributions
- Completed the Participant Donor Form as outlined above
- For your records, made a photocopy of everything you're submitting
- You didn't use any staples, tape or glue in your package to Paycor, did you? Really, please don't do that.
Paycor's Address
Ready? Mail your completed contribution form and the donations to the exact below address. We included some address labels in your Kickoff packet to start you off.
The Leukemia & Lymphoma Society
San Francisco Bay Area Chapter
Dept. #908
P.O. Box 145900
Cincinnati, OH 45250
Please don't ever staple, tape or glue your checks to the contribution forms. Paycor really, really hates that.
Tips:
- Allow 10-14 business days from the time you mail in your donations for them to be posted to your account.
- Keep a log of all of your donations received as well as copies of everything that you mail in.
- Just mail the Participant Donor Form and your donations to Paycor (without tape or staples or...). Don't send extras like sponsor forms, donor cards, donor letters, etc.—it just slows down processing and adds to your mailing cost.
- Send matching gift forms to your chapter office, not Paycor (see above).
Acknowledgements and Receipts for Donors
See Tax ID, 501(c)(3) and Receipts.
Privacy
LLS does not sell or share our mailing lists. Your donor's name will be in our database so they may be thanked for their donation and they might receive future correspondence about LLS and TNT. If your donor does not want to receive additional mailings from LLS, please block out their address on their check or do not include it on the cash donation form. (However, this also means they will not receive a receipt or acknowledgement from LLS).
Anonymous donations
If your donor wants to keep their name or donation amount private, just ask them to donate through your webpage and make sure they click "Make this donation anonymous" and/or "Hide the donation amount".
If your donor wants to remain anonymous and can't donate online, take their donation and mail it to Paycor as usual, but keep in mind that Paycor is obliged to enter all donor information that you give them (this is a good thing—your donors won't get a receipt or a tax break if Paycor doesn't know who they are). You'll need to make the donation anonymous when it shows up on your fundraising page.
To make a donation on your fundraising page anonymous, log in and go to your Donations tab, click "Show More" on the donation, then change the donation display settings.
