How to submit recommitment packages

Recommitment packages

Fill out all forms completely and return them to us before the deadline stated. If you submit your forms late, we cannot guarantee your event entry or weekend arrangements.

How to submit recommitment packages

There are four ways to submit the PDF forms:

  1. Green: No need to print out any paper! Submit the forms electronically via pdfescape.com.
    • When you first click the link, PDFescape will ask you to create an account, but you can just ignore the question and continue without an account.
    • You can load the form into PDFescape two ways: load the forms from this page, or upload the forms from your computer.
      • To load a form from this webpage, right-click on a form link above and click "Copy Shortcut". In PDFescape, click "Open a File From The Internet", then put your cursor in the URL box and hit ctrl-V.
      • To upload a form from your computer, first right-click on the form link above and choose "Save Target As", then choose a location on your computer and click "Save". In PDFescape, click "Open a File From Your Computer", then click "Browse" to upload the form from where you saved it.
    • With the form open, use the "Text" button to lay out text as you would on pen and paper. When asked to provide your signature, just type your name and the date.
    • To submit the form, choose "Save as PDF" from the File menu, and check the box for "Email PDF to me as well". In the Email Address field type sftntinfo@lls.org, then click "Save and Download" to submit the form and save a copy on your computer.
  2. Email: Print the forms and fill them out, then scan and email them to sftntinfo@lls.org.
  3. Fax: Print the forms and fill them out, then fax them to 415-625-1159.
  4. Mail: Print the forms and fill them out, then mail them to 221 Main St Ste 1650, San Francisco CA 94105.
